Follow the simple instructions on the form.

3. Provide Copies of Bill(s)

4. Submit packet

Submit an application, including supporting documents and any further comments, to cerfnh@gmail.com

5. Wait for confirmation from a CERF Trustee

Allow at least a week to process the application.
